In Southern Nevada alone, there are over 10,000 special education students living with Intellectual and Developmental Disabilities in the Clark County School District. These students will all eventually age out when they reach 22 years of age. Many of these individuals are grossly under-served or not served at all. Especially as they start to grow up, services are less and less. On a national level, more than 50,000 youth with developmental disabilities are aging into adulthood every year. The unemployment rate is as high as 80% and approximately 85% of adults live with family members who worry about the future care of their adult children when they are gone.
With the current growth in diagnoses and prevalence of intellectual and developmental disabilities, Southern Nevada has witnessed a tremendous gap in opportunities to service this group of children, youth, adults, and their families. Resources in the Las Vegas Valley are limiting, as there are long waiting lists.
PATHWAYS PROGRAM
Opening in 2021, the Collaboration Center campus will have various collaborative partners that will deliver a comprehensive family centered service model through the life span addressing many areas of need for a life filled with a higher level of functioning along with a greater sense of well-being and independence.
However, we recognize the need to service our community, starting today, so starting NOW, Collaboration Center Foundation will launch our Pathways Program. Collaboration Center Foundation Pathways Program.
Pathways Care Coordination Program (Birth-Lifespan)
Collaboration Center Foundation provides program support to help families have a cohesive and coordinated concierge experience that is welcoming and caring. We want our clients to know that they are not alone in this sometimes-arduous process of coordinating services all over the valley. This Navigation, Community Referral and Case Management Program guides individuals, family members, caregivers and providers on what resources and funding are available in our community, and at our future campus, for those living with Intellectual, Developmental and Physical disabilities (I/DD/PD). This program will be managed by a Licensed Professional and will use social work interns to help build capacity to serve more clients while gaining experience for future careers, Telehealth will play an important role in reaching out to the rural areas of Nevada.
Pathways Family Inclusion & Peer Engagement Program (Birth-Lifespan)
This program will fund wrap-around services that our community partners may not provide in the Center, such as, Sibling Support Groups, Grandparent Support Groups, Parent Empowerment Trainings, Friday Night Movie Nights, Swim Parties, and special Hair Cut days for those have sensory and attention issues.
